Help! Poking holes below cut depth

Why is my machine suddenly poking holes below the cut depth as it finishes the design?

Ensure your Z axis is calibrated properly, your Z probing is established properly, and you’re material depth is set correctly.

Can you point me to an area I can read about calibrating the z axis? Another issue is it keeps carving deeper and deeper as it goes along.

Check the set screw on the Z axis pulley.

Are you referring to these two screws?

V-bit protruding too deep, causing the dog bone effect.
Is either mechanical or V-bit related (flat tip or minor true angle variation vs stated)

Yes exactly.

Update. With the help of Julie from Inventables I was able to fix this issue by readjusting (pulling back) the z axis motor. So happy to up and running again!!
