How common are Completely random software glitches?

This is what I intended to carve, and this is what was actually carved. Immediately carved it again without changing or restarting anything and it was perfect. Completely random software glitches, just wondering how common these are?

Most likely you skipped some steps, machine lost position, and ran with it.

If a machine is not configured or used correctly these errors are very common.
It looks like you lost position on the X-axis which caused the havoc.

Pinions slipping, faulty wiring, to aggressive or incorrect feed rate/RPM/acceleration values can all add to that equation.
If the Z-axis isn’t perpendicular to the washboard deep carves can throw it out of sync since the top of the bit rub.
Those are the places to look.
